Lost/Stolen Card

If you are in need of a replacement Spartan Card, you will need to visit our office. You must bring a government-issued photo ID such as a US driver's license, passport or state ID.

  • Students

    If your Spartan Card is lost or stolen, we recommend that you deactivate your Spartan Card at the Spartan Cash website. If you find your Spartan Card prior to obtaining a replacement, you can also use this website to reactivate your card.

    To obtain a replacement Spartan Card, you will need to visit the ID Office and provide your APID and a valid form of government-issued photo identification (US driver's license, state ID, military ID or passport).

    There is a $20 charge to obtain a new Spartan Card. The charge is applied to your student account.

    Once you have obtained a replacement Spartan Card, your old card is permanently deactivated and cannot be used again. If you find your old card, you may bring it to our office and we will apply a $10 credit to your student account.

  • Faculty, Staff and Retirees

    If your Spartan Card is lost or stolen, we recommend that you deactivate your Spartan Card at the Spartan Cash website. If you find your Spartan Card prior to obtaining a replacement, you can also use this website to reactivate your card.

    To obtain a replacement Spartan Card, you will need to visit the ID Office and provide a valid form of government-issued photo identification (US driver's license, passport, state ID or military ID).

    There is a $10 charge payable by cash, check or departmental account number for a replacement Spartan Card. We do not accept credit or debit cards for replacement fees.

    Once you have obtained a replacement Spartan Card, your old card is permanently deactivated and cannot be used again.

     

If you find a Spartan Card, it should be dropped off or forwarded to our office.
